  • Come visit us!! We will be happy to set up a tour of our facility and help you determine if Chandler is the right fit for you. We strive to provide a homelike atmosphere that inspires comfort and enjoyment and only you can decide if this will be the right place for you.
  • Every individual has a different cost based upon insurance. We are a MediCare and MediCal certified facility, meaning that if you are enrolled in these programs the government will pay for all or a percentage of your fee. You may be responsible for a Share of Cost based upon diagnosis and length of stay. We are also contracted with Caremore HMO and have worked with other HMO’s in the past. We will work closely with you to determine if Chandler will work for you. We encourage you to contact our biller whose primary responsibility is to assist you through the insurance process and if necessary, to create an appropriate payment plan with you.
  • If you do not have medical insurance and are not eligible or have exhausted your MediCare coverage, you may apply for California’s administered insurance called MediCal. We will help you apply for this coverage should it be appropriate for you.
  • Absolutely. We have onsite Physical, Occupational and Speech Therapies that are committed to helping our residents achieve the highest functional levels possible. Physical Therapy consists of exercise and motor re-training techniques to increase strength, endurance, balance, and flexibility as well as to decrease pain. Occupational Therapy assists in regaining the daily living skills that have been affected by injury or illness. Speech Therapy focuses on regaining diminished communication skills as well as improving the motor function involved with swallowing following debilitating strokes.

    In addition, through our Rehab Team, we are able to offer Short-Term Rehab stays to residents whose Physician orders stays of 2 weeks or longer to get their patients safe and stong enough to return to their previous living arrangements. Therapies are available 7 days a week.
  • Yes, Chandler has contracts with various hospices to provide holistic support to our residents as well as their families during this time of life.
  • Yes, Respite Care consists of a 5 day stay so a resident can have proper care and attention while families or caregivers take a break from everyday care.
  • Pending your insurance approval, our contracted pharmacy will provide your medications and our nurses will dispense of all medications as ordered.
  • Our Registered Dietician works to create a delicious and balanced diet specific to your medical needs. We can prepare any meal plan that your Physician orders. Meals are served in our dining room or you make take your meal in your room. Assistance with eating is provided to those residents requiring extra help. Our meals are posted on this website so please check our varied and tasty menu. Unfortunately, we cannot offer Kosher meals due to preparation limitations. If you would like to bring in food for your loved one, please check with your nurse prior to sharing any food that may by contrary to doctors orders.
  • We offer free cable and WiFi in each resident’s room. Rooms also are phone-ready but you will be responsible for contracting with a phone carrier privately. We do not provide TVs or phones. New to Chandler is free Skype in our Activities Room and we are very excited to provide our residents another means of staying in touch with family and friends who are farther away.
  • Based on a Physician order, we will refer to an appropriate Doctor for evaluation and treatment. We are very involved with observation and follow-up, closely monitoring drug efficacy and physical and mental well-being.
  • While at Chandler, we hope that you feel as though you are home. Residents are free to spend their day as they wish. As needed, we have staff to assist with bathing, grooming, dressing, and eating. Housekeeping services are provided for you. We do provide a daily program of entertainment, various social and physical activities. Holidays are celebrated with decorations, parties and special meals. We are conveniently located near the Glendale Galleria and The Americana at Brand and offer regular shopping outings. Please check out our Calendar of Events to see what this month has in store.
  • We will arrange transportation to all external Doctor appointments for you and ensure that you have an assistant to travel with you.
  • Should you need assistance with any of the above, we will refer you to the appropriate Doctor and assist with getting authorization and an appointment in a timely manner. These services are offered in-house for your convenience.
  • Our facility requires all visitors to sign-in at the front desk. Both entrances are alarmed and ambulatory residents have Wanderguard wristbands that will trigger the alarm if the door threshold is crossed.
  • We are a 99 bed facility composed of 2 and 3 bedroom rooms. Should you request a single room accomodations can be made for an additional fee.
  • We have a staff of over 85 professionals dedicated to the physical, emotional, and social well-being of our residents.
  • At Chandler we have staff speaking English, Armenian, Tagalog and Spanish.
  • Our established visiting hours are from 1pm to 8pm. These hours are to allow residents ample time to rest, and complete their morning routines. However, we realize that families’ schedules are all different and we are happy to accomodate you. In special circumstances, arrangements can be made for overnight family visits. Please speak with your nurse to organize a schedule that is appropriate for your family. Additionally, all family members are welcome but we ask that you inform your nurse if any visitors are under 12 and limit in-room visits to 3 people at a time. We offer other indoor and outdoor areas for larger gatherings.
  • Pets are allowed to come visit, but not to live. We just ask that a member of our Administration meets your pet prior to bringing into the facility to ensure the safety of all of our residents.
  • You can request for your family to do your laundry or we will do it for you. We ask that each resident has 5-6 comfortable outfits and 2-3 pair of easy on shoes. Should you need clothing we will work to provide them for you. Please clearly label all personal items of clothing.
  • This team consists of the resident’s physician, psychiatrist, psychologist, social worker, nurse, activity director, dietician, and MDS co-ordinator who are responsible for working with the patient and their family to assess and create goals and treatment plan for the resident. This team meets monthly to review patient progress and evaluate and renew a plan of care.

Chandler wants to help our residents stay connected! To help our residents keep in touch with loved ones,
we offer a workstation with a large touch-screen computer. We will assist with setting up complementary Skype accounts to allow more face time with family and friends far away. Additionally, Chandler has Wi-Fi throughout and all resident rooms are cable ready.
